Description: Hospice LPN – Provide Hospice Nursing services offering emotional, social and spiritual care and support to patients and their care givers.
· Nursing: Assist the RN/Case Manager with providing hospice care in accordance with the patient’s plan of care. Assist the RN/Case Manager in performing specialized procedures as directed by attending physician and Hospice Medical Director.
· Documentation: Submit complete, accurate and relevant clinical notes in a timely manner. Assist the RN/Case Manager with the preparation, implementation, and continuing evaluation of the patient plan of care and inform the RN/Case manager of changes noted during visits.
· Communication: Assist in providing and reinforcing education to patients/family members regarding the dying process Obtain a medical history from the patient and/or a family member particularly as it relates to the present condition.
· Technology: Use of Electronic Health Record, Email, computers, and cell phone devices.
· Must be a graduate from an accredited School of Nursing.
· Must be licensed in the State of Ohio as a Licensed Practical Nurse.
· Have a valid, unencumbered LPN license, and be able to administer pharmaceuticals, perform medication reconciliation, and any nursing procedures or special treatments needed within the scope of nursing practice for state.
· Have one year of LPN experience. Hospice experience preferred.
· Have excellent assessment skills and problem-solving skills.
· Be skillful in organization and in the principles of time management and have knowledge of symptom management.
· Be a licensed driver with a reliable, insured automobile.
· Must be able to communicate well, both verbally and in writing
· Must be a graduate from an accredited School of Nursing.
· State of Ohio License – Licensed Practical Nurse (Unencumbered)
· Physical activities will include, walking, sitting, stooping, and standing.
· Travel 100%
· Able to lift fifty pounds.
