Caring for our patients where they reside
Keeping our patients comfortable is our top priority. We can increase how often we check in on them when it’s clinically necessary. Our highly skilled staff and their regular visits allow us to keep track of symptoms and head off any crises.
Short-term relief for a patient’s primary care giver
Respite care can help you as a caregiver by providing a new environment or time to relax. It’s a good way for you to take time for yourself. This can be done in the home or by transporting them to a skilled care setting.
Increased care during times of intense need
Continuous Care offers the level of support patients and families need to feel comfortable at home during crises. Our goal during these difficult events is to keep the patient comfortable and provide emotional support for the family. This creates a safe environment where families can spend quality time with their loved one.
Continued care after your loved one has passed
No one should have to walk through the grieving process alone. Our team will call and check-in with you once a month for thirteen months following your loved ones death and offer emotional support, spiritual care, and additional resources to guide you through this difficult time.
